Phew to deliver bespoke patient discharge app for Alzheimer’s Society

We are delighted to announce that we’ve teamed up with Alzheimer’s Society’s Innovation team to pilot a custom module for the Patient Discharge Management System (PDMS) which will enable families to plan for their loved one’s discharge from hospital.

The role of Alzheimer’s Society’s Innovation team is to identify and develop new solutions to help everyone affected by dementia to live well. One of their current projects is improving patient care with a newly developed hospital journey tracker to help reduce the amount of time that people living with dementia spend in hospital unnecessarily and offer transparency of patient’s care.

Phew’s PDMS provides healthcare professionals with up-to-the-minute information about patients’ discharge status and has been proven to reduce patient’s length of stay and enhance patient experience. As the PDMS already contains patient data, it was clear that the system provided an ideal solution which could be used for trialling the hospital journey tracker.

The first stage of this project is to create a bespoke module for the PDMS which will provide patients and patient representatives with email status updates, informing them of what is happening now and what is expected to happen next in a patient’s care.

The team at Phew have already begun the work and it is due to go live on the system by the end of February 2022.

Natasha Morgan, Senior Innovator at Alzheimer’s Society, commented: “This is an ambitious project for us as building technology for the NHS is notoriously challenging. Phew’s expertise and existing connection to Luton and Dunstable University Hospital means they are perfectly positioned to make the hospital journey tracker a reality. We are determined that by working together we can help enable people affected by dementia to leave hospital safely and without delay.”

Matthew Burgess, Managing Director of Phew, added: “This is an incredibly exciting project, to improve patient outcomes. Many of us have experiences of loved ones with dementia and any support for carers during these times can only be of additional help.

“Knowing the impact that the PDMS has made to patient outcomes and budgets in both Acute and Community settings, we’re really excited the see the results of this project with Alzheimer’s Society.”
