Development update on bespoke patient discharge app for Alzheimer’s Society

Last month we announced that we’re working with Alzheimer’s Society’s Innovation Team to pilot a bespoke module for the Patient Discharge Management System (PDMS) at Luton and Dunstable University Hospital, with the aim to help reduce the amount of time that people living with dementia spend in hospital unnecessarily.

A key aspect of the project is to provide transparency of a patient’s care, so the new module will provide patients and patient representatives with email status updates, which show what is happening now in a patient’s care, what is expected to happen next, an anticipated discharge date and when the patient was discharged.

The team have been working hard behind the scenes to develop the pilot module and we are excited to share images of the Patient Status Update and the Patient Discharge Status emails.

The patient tracker within the PDMS has also been in development so that healthcare professionals will be able to allocate a patient with dementia this status within their record and report on what is happening now and next in their care. They will also be able to review the total number of dementia patients, their average length of stay and how many email status updates have been sent out.
